Individuals – Instructions to Get Started 

  • Contact us for an appointment, or to inform us of your forthcoming information. E-mail and/or telephone are fine if no in-person appointment.
  • Download the appropriate individual tax organizer and complete it to the best of your ability. These forms will help identify income and potential tax deductions, and help organize your tax information:
  • Gather applicable tax forms and statements:
    • Forms W-2, 1098 series (Mortgage Interest, Student Loan Interest, Education Tuition, etc)
    • Forms 1099 series (Interest, Dividends, Brokerage, State Refunds, Unemployment, Social Security, Debt Cancellation, etc)
    • Forms 5498 (IRA) and HSA/MSA (Health Account reporting)
    • Schedules K-1 (From Partnerships, S Corporations, Trusts, and Estates)
    • Health insurance Forms 1095-A (Mandatory, if applicable), B, and C
    • Charitable cash donations exceeding $250 and non-cash exceeding $500 (Mandatory)
    • Proof of state income tax and/or sales tax paid, real estate tax paid
    • Final escrow settlement statement(s) for purchase or sale of real property
    • Final escrow settlement statement(s) for ALL refinanced mortgage loans on currently owned real property
    • Provide a copy of current state issued identification (e.g. Driver License, State or Military ID, Passport/Visa, etc)
    • IRS and/or state issued letters reporting benefits received, e.g. advanced child tax credit (ACTC) and economic stimulus payments received, etc
  • New clients: provide a complete copy of your last filed individual income tax return and, if applicable, most recent fixed asset depreciation schedule
  • All tax paperwork and forms can be sent in Adobe PDF file format via the Onvio© client portal. You can also deliver the tax information by mail or in person appointment

Businesses – Instructions to Get Started 

  • Contact us for an appointment and to request bookkeeping write up services. E-mail and/or telephone are fine if no in-person appointment.
  • Download the appropriate business tax organizer and complete it to the best of your ability
  • Documentation should include copies of new:
    • Mileage log summary and ending inventory value
    • Promissory notes and shareholder loans
    • Equipment and property purchased or sold
    • Leased equipment and property
    • Financial documents and agreements pertaining to the business
    • Annual Meeting Minutes and stockholder/equity changes
    • If you have employees or independent contractors, please provide:
      • IRS Forms W-3 and W-2 (for employees)
      • Federal and state payroll reports (e.g. Forms 940 and 941, Oregon OQ, etc)
      • Forms 1099 and 1096 (for independent contractors)
  • Bookkeeping clients: we use the Quickbooks Online platform (“QBO”) for all financial write-up. We have discontinued use of the Quickbooks desktop version. Financial data is compiled in proper financial statement format for income tax reporting.
  • New clients: provide a complete copy of the last filed business income tax return and most recent fixed asset depreciation schedule.
  • All tax documents and forms can be sent in Adobe PDF file format via the Onvio© client portal. You can also bring the tax information with you to our appointment.
